To obtain the permit you will need:
1. A copy of your birth certificate with a raised seal or notorized stamp on it.
2. 2 forms to show proof of residency. One form can come from a parent’s driver license if address is current. Illinois ID card can be used, as well as a bank statement or grades that have been notarized by the school office.

4. A State ID card may now take the place of a copy of your birth certificate. It will also count towards a proof of residency unless your address has changed.
The student must have a white slip for at least 9 months and 1 day and 16 years of age before being eligible for their driver’s licenses. A Student, age 18, is legally able to get his/her state license without taking driver education. They must still take the State test and pay the fee. A white slip is valid for 2 years from the date it is given.
